• SIMPLE  (BASIC)  WILL PACKAGE  This package is ideal if you have only a few assets and no minor children. It includes a Basic Will, General Durable Financial Power of Attorney(s), Healthcare Directive(Living Will/Medical Powers of Attorney), HIPAA Authorization Form, Burial Instruction, and Supporting documents). 

• FAMILY WILL PACKAGE:  This package includes all the features of the "Basic Will Package", plus one "testamentary trust" provision.  It is ideal for families with minor children or grandchildren to whom they wish to leave a gift or adult children to whom they want to leave assets in a testamentary trust (non-SNT) until they reach a specified age(s) or other milestones.    

• REVOCABLE LIVING TRUST PACKAGE (Probate Avoidance): This package can avoid probate,  guardianship, and conservatorship. It includes a  Revocable Trust, Declaration of Trust, Certificate of Trust, Pour-over Will, Durable (Financial) Powers of Attorney, Healthcare Directive(Medical Power of Attorney/Living Will), Instructions for Retitling Assets into Trust, Burial Instruction, HIPAA Authorization Form, and Supporting Documents. Distribution is general (*additional charges for special gifts.).  

• SPECIAL NEEDS WILL PACKAGE.   This package is ideal if you have children or beneficiaries with disabilities ( i.e., educational, learning, physical, and mental). It can protect their rights to receive income and assets from you, relatives, and friends without rendering them ineligible for public benefit programs (i.e., Medicaid, SSI).   It includes a Basic Will, General Durable Financial Power of Attorney(s), Healthcare Directive(Living Will/Medical Powers of Attorney), HIPAA Authorization Form, Burial Instruction, one (1) special needs trust, and Supporting documents.

• SPECIAL NEEDS TRUST  PACKAGE .    This package is ideal if you have children or beneficiaries with disabilities ( i.e., educational, learning, physical, and mental). • PET TRUST PACKAGE: This package is ideal if you have a favorite family pet(s) you want to provide for if you become incapacitated or die.   With a pet trust, you can leave money to be used to care for your pet. You put someone in charge of managing and spending it, following a written set of instructions that you provide.   

• DURABLE POWER OF ATTORNEY PACKAGE:  This package can help avoid a lengthy and costly guardianship/conservatorship proceeding.  It is essential for adults, including elderly parents and relatives, high school and college students(18 years of age and older), and other unmarried adult children.  It includes a General Durable (Financial)Powers of Attorney, Healthcare (Medical) Power of Attorney, Healthcare Directive(Living Will), and HIPAA Authorization Form.  Individuals who make this package should also consider making a "Will" or "Living Trust" if they do not have one.

• STAND ALONE SIMPLE  WILL( simple will only). Most people need additional documents for complete protection, i.e., living trust, financial power of attorney, healthcare directive, and HIPAA Authorization Form.

• CODICIL TO WILL - allows you to supplement or make changes to your will (i.e.,  change executor, update beneficiaries, update gifts to a beneficiary, update guardian selection for children under 18). Ideal if you have only a few minor changes.

• GENERAL DURABLE (FINANCIAL)  POWER OF ATTORNEY - gives the person you designate (Attorney-in-Fact) the power to make financial decisions on your behalf (i.e., paying bills, filing your taxes) when you cannot.

• HEALTH CARE DIRECTIVE (MEDICAL POWER OF ATTORNEY & LIVING WILL ) - gives the person you designate (agent) the legal authority to make healthcare decisions on your behalf if you cannot do so because of incapacitation.    

Price: $249.99  (Individual)

• CUSTODIAL POW ER OF ATTORNEY -  is ideal for parents who may need assistance caring for a minor child. They can give another person, i.e., parent, friend, etc., authority to take care of a minor child when hardship or other factors prevent them from doing so.

• BENEFICIARY (TOD)DEED -  when timely recorded, can avoid the probate court process and transfers title to your real estate to your designated beneficiary (including your revocable trust) upon your death. Your real estate remains titled in your name during your lifetime, and you can change or revoke it before your death.   Individuals who make this document should also consider making a "Will" or "Living Trust" if they do not have one in place.

Price $299.99 (Single Deeds), plus recording cost.  If you own other real estate, we can draft additional deeds for $100.00  per beneficiary additional deed. Also, we will provide you with instructions on how to record the Affidavit or record it for you for an additional charge of $100.00 per recording.

• AFFIDAVIT OF DEATH/AFFIDAVIT OT ENTIRETY - recommended if the person(s) you hold real estate with as a "joint" or "tenant by the entirety" preceded you in death.  It  removes a deceased owner from title to property by recording evidence of the deceased owner’s death in the land records. A survivorship affidavit clarifies the land records by letting third parties—title companies, lenders, and property tax officials—know that an owner has passed away and that you now own the property without that owner.  It removes the name of the deceased persons(s) from the title.

• AFFIDAVIT OF HEIRSHIP - can establish the legal heirship of an individual who dies without a will or in some instances, with a will.

• SPECIAL GIFTS - transfers specifically identifiable property(i.e., my car)  to specific individuals or organizations ( i.e., John Doe; ABC Charity, etc.) A general gift is a specific or group transfer (i.e., to my children, parents, siblings, etc. ) of non-specific property (i.e., my estate).
